Homer later releases all claims on the baby, Angel, and devotes his attention with renewed vigor to his practice with Dr. Larch. Eventually he takes over the orphanage and practice after Larch's death. He steps right into the role of his former caretaker, continuing the man's legacy. He is Homer's father figure, the head of St. Cloud's orphanage. After a particularly disturbing encounter with a prostitute as a young man, Larch remains averse to women and familial relations.

The story, set in the pre– and post–World War II era, tells of a young man, Homer Wells, growing up under the guidance of Dr. Wilbur Larch, an obstetrician and abortion provider. The story relates his early life at Larch's orphanage in Maine and follows Homer as he eventually leaves the nest and comes of age. Homer gets a job picking apples, living in barracks with migrant workers led by Mr. Rose (Delroy Lindo). On the wall is a list of rules, but the migrant workers cannot read, and believe that since they didn't write the rules, the rules cannot apply to them.
